Physical Examination

An updated Preparticipation Physical Evaluation form must be on file for each student participating in a sport. All athletes must have a current physical submitted that is not older than 12-months. Athletes with expired physicals must submit new physicals to the coach of their respective sport before they participate in any tryout or practice.
